Application for Aid
Before submitting an application for assistance, please read the following information about our process and gather ALL required documentation.
All questions MUST be answered. Incomplete applications will NOT be reviewed.
Fairy Tail Endings is a small, all-volunteer nonprofit organization that relies almost exclusively on private donations to help pets like yours. We all work full-time jobs and are not able to respond to emergencies outside of our ER veterinary partner referrals.
We receive numerous inquiries and requests every day. Due to limited funding, not all qualifying applicants will be awarded assistance. Funding decisions are made at the discretion of Fairy Tail Endings (FTE) and based on: fund availability, financial need of owner, the opinion of the treating veterinarian as to the medical necessity, urgency and prognosis for the treatment needed, or situational urgency for non-medical care.
Applicants must be Sarasota or Manatee county residents & income-qualified.
While we actively promote pet rescue/adoption, FTE only funds the care of owned pets that are not being re-homed and have been part of your family for at least 1 year. Please see our 'More Resources' page for links to additional local and national programs that may better match your situation.
FTE does not fund the care of puppies or kittens under 1 year of age that have been purchased through a pet store, breeder or other venue such as Craigslist or pets that have been added to the family after financial hardship has been demonstrated, except under special circumstance (ie. certified service/therapy pet).
Applicants must agree to spay/neuter unaltered funded pet(s) within one month of treatment. Proof of surgery will be required and a FTE representative will follow up to ensure compliance. Referrals to no/low-cost clinics will be provided. Applicants must also commit to keeping their funded dog on heartworm prevention. Heartworm disease is endemic to our area but nearly 100% preventable with a simple, affordable monthly chew. This protects our investment into your pet's long-term health!
Proof of income and/or temporary financial hardship is required & MUST be submitted with your application to be considered. (i.e. 2018 Tax Return, 1 month of paycheck stubs for all adults in household or benefit determination letters for government assistance programs). Failure to provide proof of qualifying income after treatment may result in legal action to recoup funds expended and any associated legal fees.
Our assistance is a one-time gift meant to help your pet through a medical crisis, or provide diagnostics & initial treatment for a chronic condition with a good prognosis, that you can maintain the cost of once baseline health is restored. At this time, we are unable to fund MRIs, neurosurgery, cancer treatments such as chemotherapy, long-term or hospice care.
Initial correspondence is via email except for life-threatening emergencies. We check our email frequently, please do the same.
Applicants that are not on government assistance and all ER clinic clients are required to apply for Care Credit. While no one wants an extra bill, our assistance is reserved for those applicants who have no means to get needed vet care. You can apply 24 hours/day at www.carecredit.com. Even if you don't believe you will be approved, we ask that you try and save a copy of your decline letter to submit.
FTE can not fund care that has already been performed or past balances on your veterinary bill.
